Student Instructions for Obtaining a Work Permit
1. Complete the Student/Application Information section. Parents must sign this section. (Superintendent signature not required)
2. Have your employer complete the Pledge of Employer section - the permit cannot be issued if the Employer's Tax ID number is missing.
3. Have a physician complete the Physician's Certificate for Minor Work Permit. If you have an athletic physical form on file at the school that is less than a year old, mark the appropriate box on the Student/Application Information section.
4. Return the form to Mrs. Richard in the guidance office. Once she enters the permit information she will contact you to pick up the permit, sign it and return it to your employer.
5. Each change of employment requires a new permit.
